Poor Service at Casualty

My Grandson was taken to the Linksfield Casualty on Saturday evening with a temperature of 39 degrees he is 3 years old. He was given medication and sent home.
Firstly we were not asked if we have recently travelled, no urine test was done

My understanding is that a Paediatrician should have been summoned to him also that it is standard practice a urine test should be done. Bloods were taken and an x-ray done. This x-ray could not be found when his doctor requested these, he was finally admitted to hospital on Tuesday due to a continued temp of 39 degrees

This is the second time we have received very bad service from Linksfield Emergency as two years ago my son was flown from the DRC very ill and we took him to the Linksfield ER, bloods were taken and he was told all clear and he could return to the DRC. On his return to the DRC he was so ill he lost consciousness and had to be flown back to Johannesburg. It was found the bloods clearly indicated he had Hepatitis B
I contacted the Owner Doctor who told me this was a human error

Does this emergency service run on continual human error when people's lives are at risk
