Refusal to pay for what they requested

Fedhealth requested a "motivation" letter from my surgeon after rejecting a request for approval for an operation to remove a cancer growing in my arm, which is covered by my policy. My surgeon submitted a written motivation for the operation, which was then approved, and added his time for writing the motivation on to the account with ICD code etc. Fedhealth has refused to pay this amount stating that it is not covered by the scheme. THEY requested (ordered) the letter, I did not need a motivational letter to prove that I had a cancer growing. Surely whoever orders something should pay for it.
