No feedback since 11 july AND bill are not paid

My daughter had to go for a MRI scan in Feb, 6 months later we get a account from the dr's to say we have to settle the account??? From our side we did get Authority number for this MRI to be done, the account was submitted to you for payment, full amount was not paid, no reason was given?
After 4 months its apparently a "stale" account now and balance will not be paid that was supposed to be payed in full the first time? the Dr did continuously submit this account for payment to no avail.
I can't see how we must be now hold accountable for this outstanding balance.
1. We did get authority number for this and it was authorized.
2. Account was submitted to you but was not paid in full, no reasons where supplied.
3. There was continuously follow up from the practice, still no payment.
4. Now after 4 months its all of a sudden a "stale" account.
Can we please sort this out, we have been members for a long time now without any problems, can we please keep it this way.
we are still waiting for a reply and response since 11 july.this is TOTALLY unexeptable
