Negligence of accident report by Peglarae Hospital

My mom was involved in an accident on on the 30th October 2010. Since November 2010 to date She have been asking for his accident report from the hospital (Peglarae) but the gentlemen in charge kept on telling her that the attorneys had to pay R600.00 before getting the report. The attorneys requested for that report several times but till today they haven't got it yet.
Ultimately this year when she went to the hospital to request for it, the gentleman told her that he couldn't find the file. On the 05 July 2013 she asked me to assist by communicating with Theuns de Bruyn he always tells me that the file is missing i must call later when i call he tells me that he is busy serching on the archives container. On the 31 July 2013 my mom wrote a letter to the honourable CEO requesting assistance to get her accident report. She also attached copy of letters from Edward Leonard Ndzabandzaba Attorneys.
On the 06 August 2013 i received a call from Mom's lawyers informing me that only 2 weeks left, if my Mom doesn't send accident report her file will be closed
