Farcical Allied Therapy "Approval"

Discovery, I pick up where we last left off 23 April 2013, WRefNo#2816494020#:-

I still await a satisfactory reply to my questions:
Kindly point out where on the application form a "clear and concise" deadline was stipulated - I unfortunately don't see anything - had I, I would not have put all my time into this... Benefits apply on an annual basis - the whole of 2012 - not some arbitrary, after-the-fact cut-off date, surely?
And:
Your feedback below: "Based on the clinical recommendation of the South African Society of Physiotherapy, Discovery Health will pay for an additional 6 Physiotherapy sessions." To reiterate, I am puzzled as to how I am to squeeze in another 6 treatments - between 2011/11/14 and 2012/12/31 - while out of 9 prior treatments in the year, Discovery is only covering 4 (over eleven months)! Please provide me with the reasoning (rational) that lead towards your conditional conclusion.
Per above, please explain how, according to Discovery, 6 treatments in 1.5 months are acceptable, but not more than 4 treatments over the 11 months prior?

This is now my TENTH email. My patience are wearing very thin.
