Manager refusing to claim

Im employed by Cashbuild and I sustained an injury to my spine whist lifting heavy objects during the course of duty. I went to the doctor and he sent me for xrays. Xrays show that from heavy lifting my spine has twisted in an s shape. The pain is extermely bad, my doctor has prescribed many different pain killers but the pain does not stop. I gave the claim form to my manager and he refused to claim for me. Please help me as it is affecting my life. I need to get my back fixed. I have all the papers, letters, invoices and evidence that it is injury on duty.
