Driveway breaking up after only 2 weeks

Driveway was tarred in May 2013 and began to break up soon in
June, after been told there was a 5 year guarantee on the work we tried to contact
Lloyd to come and repair the driveway but now 5 months later he has still not repaired the driveway and now refuses to take any calls.

Even when contacting the JHB number there has been no feedback.
