Getting UIF forms for maternity leave

I called Quest Joburg first week January explaining that i used to work for quest and i went to labor department and they want certain forms from quest because i want to claim uif as i am on maternity leave. The lady that i spoke to took my email address ID number cellphone number and said they will email my forms. I waited for a week and called again after not getting any reply.

When calling for the second time on the 15th Jan. I was given an email address [Email Removed] they told me to send my email address, ID number and cellphone number and said they will email my forms and still i got no reply

then called again on the 22nd Jan. And they gave me a different email [Email Removed] and they told me to send the same things my email address ID number and cellphone number. And still haven't heard anything from quest.

I am now on maternity leave and i am broke i don't have money to call quest because i am on unpaid leave. Can someone from Quest send me the forms that i need to take to the department of labour because now they cant process my application because of outstanding forms from Quest.
