Shocking service

Herewith are all the elements of our evening that went wrong:
Drinks order was taken only 20 minutes after we were seated
Food was ordered and waitress just walked away without taking our menus
Once food arrived correct cutlery (such as steak knives) were not brought to the table so we needed to wait
Meals arrived and the steak which was ordered to be medium rare was brought well done. When the food was brought back no steak cutlery was replaced. A used steak knife was used as we just didn't want to wait for the meal to go cold before we even saw our waitress.
More bread was requested and not brought (table was originally only 2 people and grew to 8)
A 500g rump was ordered yet the waitress brought a t-bone- this needed to be sent back so that we could get what we ordered
A creme br? l? e was ordered as the customer wanted it as their main and this only arrived an hour after order placed
Plates where not cleared for nearly 30 minutes
When the bill was requested we received the wrong bill all together- elements were add the the correct bill which hadn't actually been ordered by us
Deputy general managers attitude wasn't one of trying to improve the situation at all
