Car finance to 60 year old with out credit life

My father took out car finance on a 2008 model 1.5 TL chev aveo in 2012 that time he was 62 years old it was approved
He past away in 2013 i would just want to know why there is no credit life on the policy we was under the impression it will or was on there, till the trust came back and said the car amount is still outstanding and now they come with stories if someone want the car they must get finance to keep the car
I would like to know who is responsible for this and how he got the car with out the credit life
