Four months to issue a Garage card with wrong name

It has taken Standard Bank more than 4 months to issue an additional garage card for our company, and when it finally arrived it was in the wrong persons name.
After the application had been submitted to the bank, the bank required additional supporting documents, the required documents were submitted within a day or two, then after a couple of weeks they discovered they needed some more documents, this has been going on for 4 months. Centurion branch phoned to inform us the card was there, when we went to collect it the bank clerks could not find it, after the 3rd trip they finally found the card wrongly made out to a company director who already has a card for more than 15 years.
I have been a Standard Bank client for the past 35 years and the service I have received the last 2 years is regrettable- Standard Bank is moving backward instead of forward
