Pension payout

I filled out my pension payout in the end Oct with my payroll and they sent to Alexander Forbes. Now they keeping sending me the same form to complete and say they can't do my payout as there is no payment option selected. I even filled my banking details. The payout takes 4-6 weeks and its already 6 weeks. They saying that once I complete the form. It will take another 6 weeks. This is totally shocking and dissapointing.
