I cancelled my policy but debit orders are still continuing

I lost my job, and applied for premium break from May 2013 to October 2013, End of October i went to Kuruman branch to cancel the debit order as i knew that October is the last month for premium break, and to avoid bank charges, in November i cash up my policy because i could not afford to pay it anymore. To my suprise old mutual debited my account again end of November, who must pay for the bank charges cos im being charged R115 for every debit order that return unpaid.
