Threatened for payments that the BANK neglected

I have been a loyal customer with Standard Bank for more than 30 years.
I am now at the last straw as they sent me a letter of demand for payments on my (very small R7000) credit card balance which THEY NEGLECTED to take from my cheque account and their reason is THE DEBIT ORDER was cancelled (I DIDN'T cancel ANYTHING, so it MUST BE THE BANK!!!).
When I queried this this morning I merely get told "sorry, it is your responsibility to make sure the payment goes off".
Now sorry if I am mistaken but IF I INSTRUCT THE BANK TO TAKE THE PAYMENT AMOUNT DUE EACH MONTH BECAUSE I AM PUTTING THE AMOUNT ON MY BUDGET FACILITY FOR THREE MONTHS I EXPECT THE BANK TO TAKE THE MONEY EVERY MONTH, not send me a letter of demand because THEY failed to collect THEIR money.
Then the letter states "despite numerous reminders" - I HAVEN'T had ONE REMINDER!!! So please send me the proof of the reminders???
Secondly they suspended my credit card without informing me, leaving me in the lurch when I needed them most, to pay for the hospital room where my wife was giving birth to our firstborn last week.
THANKS VERY MUCH STANDARD BANK!!!
I am changing banks TODAY!! I'M FED UP WITH STD BANK'S INCOMPETENCE!!
