LOST POLICY DOCUMENT

I used my life insurance policy as security for a student loan. The loan has now been repaid and after numerous phone calls I was eventually informed that my policy document "has been lost". I get no feedback from the bank. I asked for an official letter giving an explanation - no response again. This is totally unacceptable. Isn't it time to get your act together? Documents CAN'T "disappear". Someone is responsible. It needs to be found and an explanation supplied as to why it "got lost" in the first place.
