Provident Fund manager ignoring my requests

I spoke to the Provident fund manager (Angelo George) numerous times requesting an IT3a form be sent to me. Every time he tells me there is no problem and that he will send it to me now. I haven't received anything. In the last 3 days I have made numerous phone calls and sent numerous mails to get a status update. When I do get to talk to someone, they tell me the above mentioned individual is never at his desk. My mails are now just ignored. I feel like my money is being held ransom.
