Card issuing mistake that was never reversed

On the 8th of April I recieved a SMS that a new card had been issued to me. I did not request a new card nor was my card expired.

The following day I visited the Melville branch and a friendly woman named Charmaine immediately looked into it. A card had been incorrectly issued on my account. She immediately contacted the person who issued the card but she was away at a conference, her manager promised to have the issue reversed. The card was then cancelled on the following day.

However when I went back to the branch a week later to obtain a bank statement I noticed that the charges were not reversed. Charmaine once again contacted this lady. It turned out a lady in the Mokopane incorrectly issued the card and did not even have full knowledge of her error. She promised to immediately revers the charges. It is now May already and the charges have not been reversed.

I live in JHB, Mokopane is 3hours away from here. How do you manage to issue a card on someones account without checking their details correctly and then take almost a month to fix it.

I AM REALLY NOT HAPPY WITH FNB'S SERVICE. BETWEEN NOT REPLYING TO COMPLAINTS ABOUT UJ CAMPUS ATM AND NOW THIS CARD MIX UP.
