NO RESPONSE a week later despite 48hr promise

I don't usually complain about bad service, but this time it is necessary that I do.

I am an existing Neotel customer who applied for an Internet service in addition to the voice service that I have, and was promised delivery of my Internet modem within 48 hrs once I've submitted all the required paperwork, which I then promptly did.

IT IS NOW MORE THAN A WEEK LATER and I am STILL WAITING to hear anything, with no chance apparently of receiving the device in the foreseeable future AT ALL. When I phoned customer service yesterday, I spoke to the same (helpful) sales lady who handled my application initially, and she said the holdup is with getting my documentation RICA'd. Surely it can't be that hard to RICA me again, considering I am an existing, paying customer who has been RICA'd by them before and regardless I have supplied the documentation AGAIN as requested.

WHY IS THIS TAKING SO LONG??? WHAT IS BEING DONE WITH MY PAPERWORK?

I demand an immediate response on the status of my application and a resolution thereof, otherwise I will have no choice but to take my clientele elsewhere. THIS IS UNACCEPTABLE, Neotel.
