Complain on my MTNSP insurance about my lost phone

I lost my phone on the 21 Dec 2013... Called the MTNSP they blocked my phone together with the sim card got the reference nr and al. they told me to report it and all, which I did. but da most irritating part of this whole is that when I try and call that bloooody insurance always get cut, it keeps on ringing and ringing until I get cut. what irritates me the most is that ive emailed my claim form knows how may times... faxed in Three times already... bt their machine keeps on telling " we have not receive your claim form please fax it to 011 912 5122"
so now I don't know what more to do because now when I go to MTN Shops near me they cant assist me. please help... I cant be paying for an insurance when it can meet my needs when it suppose to. they il rather terminate my contract and I wnt my refund in full!
