Blacklisted Incorrectly, and blocked my NEW phone

My previous BlackBerry Z10 was in August and reported. I then replaced the device through my FNB Insurance, where the phone was bought, I paid R1300 excess and on the 4th of September by new BlackBerry Z10 was delivered to me. I've used it for a month and a half and last Thursday I got an error message on my phone, I phoned Vodacom (which I use), never used 8-ta, and Vodacom told me that my new device has been Blacklisted by 8-ta (Telkom Mobile). On Vodacom system it shows - against my name. How the did this happen, now I have been without a cellphone since Thursday and I cant get through to Telkom mobile to sort this out, they are not answering the phones. I keep on emailing as well.

I will not stand for this, this is MY new phone, how can they list me as and blacklist my phone. I have all the proof from FNB that I claimed a new phoned through my insurance
