Deal made under my name

According to the statement i recieved end of june it showed that july 25th i'll be done paying my contract. when july came i recieved a statement showing that i had an oustanding amount of R500.00 and some odd rands and i was credited with R200.00 airtime which i queried via telephone and i spoke to a lady by the name of Thandeka who told me that i upgraded and i obviously didnt she said she would log a call for invistigation REF num 101496827and she will get back to me she never did. i then calling MTN and spoke to a guy Siphumelele was his name who was able to tell me where the upgrade was done, he told me that it was done at Meadowdale Hyperama on the 7th of june i upgraded to a S4. he then advised me to do an affidavid and certify my ID copy. i then called the MTN shop and Spoke to Debra who told me that they do not have me on their data base of upgrades and she will assist me in getting to the bottom of this. i went to the shop and submitted the affidavid and certified copy ID. this morning 23/july/2013 i recieved a statement saying that i have a balance on R1120.99 and i do not understand why. i cant afford to pay so much money for a phone i dont even have.
