Contract Cancellation - Incompetent service

I requested my membership to be cancelled.
My cancellation (ref. # 23093) was confirmed in writing and sent to me via email by Chrislynne Baker in July 2013. After some time I noticed that this is clearly not the status of my membership!!! I am still being debited for this membership. I then had a telephonic discussion with Imelda at Planet fitness Hennopsview on 4 October 2013 about the refund. Imelda then told me to send my banking details which planet fitness already have as this is the account that was wrongfully debited, to Chrislynne Baker at Planet fitness Hennopsview for the refund to take place. I did email this to Chrislynne Baker on 4 October 2013. It seems that nothing has been done after I sent the email to Chrislynne as requested!!! I then received an email from Louisa Sithole requesting my banking details again!!! I then replied to Louisa Sithole's email on 10 Oct. 2013 and explained to her the above without including my banking details as this might trigger another person asking me for my banking details from planet fitness which I have already sent to Chrislynne Baker. I have not received any feedback since then.
