Request for settlement lette

My sister needed to apply for a loan at the bank and needed her account settlement letter. she requested the letter last year, November 2012 and never received it. March 2013 made another request, and was told it would be emailed to her. An incorrect email address was used, she then phoned the head office (0861002233) and spoke to Alfonzo and Lee who advised to fax the letter in a few hours, till today she has not received her letter.
