Suspicious behaviour related to card skimming

I have been the recent victim of card skimming, and after re-tracing my steps, I have no doubt that there is something dubious taking place at the Jet in Sandton City. This is not the first issue I have had. A few months ago, my details were used to open a MTN account. The matter was sorted out by the service provider, but they confirmed that this was likely opened by someone working in a "clothing store" as they gave their description as "Fashion Store Manager". I dont have ANY clothing accounts, and the only details I have ever given out have been in the Jet store when I was offered a loyalty card, and happened to fill in the forms!! In the recent incident which took place this weekend, I have no doubt that my card was skimmed at the same Jet (sandton), as it was the only shop where I used my cheque card. They pulled the "this machine is not working" story, and I was asked to use another one. When I think back, the cashier was acting very suspiciously, but I didnt figure it out then. I suggest that this store be thoroughly investigated, as there is NO WAY the two incidents could simply be a "coincidence". I require someone senior to contact me in regarding this ASAP.
