Totally incompetant

Hi i went into woolworths illanga mall Nelspruit about 4weeks ago to get a card for my wife to use as she cannot use my card, i was told to bring her and our id books and we would instantly be given a card, on doing this we was told there was papers to be filled in and it would take a week, i did this three weeks ago and after phoning woolworths customer care i was told no forms were recieved, this is total incompetance by illanga mall Woolworths all i want is a extra card for my wife to use, i owe nothing and i pay it off every month in full. This is totally unprofesional, all i want is a card for my wife to spend money if she is not. Issued one i am not doing another 100km trip to fill forms again i will just be shopping elsewhere in the future
