Insurance Claim allegedly cancelled by me

I bought a phone for my brother in August 2012 at Edgars Vaal Mall. The lady offered me insurance on the phone and I accepted. My brother's phone was in December 2012 and we reported this to the police and Vodacom. We advised Edgars at Vaal Mall and they advised that they would call us back with feedback. More than a month later with no feedback from Edgars, we decided to follow up. I was advised that I cancelled a policy on 4 January 2012 and activated the insurance on this phone on 26 August 2012 but later cancelled. I have requested a copy of the instruction that I allegedly sent to Edgars requesting a cancellation of the insurance but have received nothing. I also have no idea what this insurance was that I allegedly cancelled in January 2012 as no one can tell me what this was for. Please urgently get back to me with an answer on what is going on here as I need my brother's phone replaced and need to know what insurance I cancelled in January 2012.
