Account not closed

Hi, please can you assist me in this urgent matter.

I've made my last payment on the 1st April 2013 thereafter I've closed my account. I've filled in a form at Centurion Mall Branch and according to my knowledge they send it through as the guy told me he did and my account is been closed.

Today I found out that this has not been done and they don't answer the telephone.

Please can this matter be sorted out immediately and take off any costs there is on my "closed account".
