Unapproved Debit Orders from Planet Fitness

I cancelled my contact with Planet Fitness due to debit orders being debited on the wrong dates.

After my cancelation letter in 2008 they stopped the debit orders. 5 years later in 2013 June month they started putting debit orders trough as many as 3 times a day!
Not only do these go off multiple times a month but the amount also varies.

After contacting them regarding this matter, they still have not stopped the illegal debit orders and can't tell me where i can have this addressed.

This is affecting my clean credit record and score rating very badly! It's plain out!
