Unauthorized Debit orders on my account

Please can you rectify the problem below. The following debits have been running on my account and I did not sign up for any of these products or offerings.

INSURANCE PREMIUM FUNERALPLNEM-3380920-4 9413 R 49.00-
SERVICE AGREEMENT ISPB EM-3380920-2 9257 R 29.00-

Please can you cancel and process my refund. I can only trace back from April 2013, but I'm sure you have a better view as to when you started running these debit orders off my accounts.
Please rectify ASAP.
