Fradulent upgrades on my Vodacom Account

My contract on MyGig 5 Promo@R299 24M expired on 5 Sep 2013, I requested to contract to be cancelled and the number to be migrated to Vodago Prepaid. This was going to be effective on 31 October 2013. In November I called Vodacom to confirm whether the cancellation was done, I discovered that my contract had been upgraded in Polokoane and I live in Kempton park. I faxed through an affidavit and id copy so that an investigation could be launched. I had to constantly call Vodacom to ask for the investigation status as they gave me no feedback whatsoever. At the end of the investigation I expected a refund of roughly R4000 based on all the debits that were done on my account. I received an sms stating my credit note is R447.36 excl. VAT. I called to query this only to discover that two additional phones where taken again on the same account so I have 3 numbers taken under my account. To make matters worse they still debited my account for January after they told me that the debit order was removed until the end of the 2nd investigation for the other two numbers. I reversed the transaction with the bank and they said my account is in arrears.
