I got this on 05/06/2013 at the CellC Blue Route Mall, it was Rica'ed almost a week later. It suddenly expired 12/12/2013, and on Cell C's system it showed that it was Rica'ed 12/12/12 by a person with a different surname.

I tried to get it fixed at Somerset Mall (CellC) with the documentation, but they told me it must be investigated. I have not heard from anyone.
