Phone blacklisted and sim block BUT STILL IN USE

My son phone was and he telephonicly reported this, hoping the sim and phone could be stopped and blacklisted. After a lenghthly telephone conversation and airtime being used the sim was however blocked and only to be told that the sim should be in the phone for it to be blacklisted.

Kindly advise why this could not be done - and why he was advised then to go online and download forms for his phone to be blacklisted. There was not even an option given for this form to be emailed either???

Please forward this form to myself to assist my son in blacklisting his phone. Seeing that he did mention that it could be picked up that someone is using his phone currently.

Herewith his phone details if there would be a slight oppurtunity for his phone to be blacklisted please.
