Struggling to Blacklist my account

Good day. For the last two months my wife has been struggling to blacklist our lost Samsung Tablet. We took out a contract with Vodacom Chatz last year December and had account issue with them. We could never used the tablet as the query was never resolved. We lost the phone 2 months ago and when we tried to block the sim card and to blacklist the phone no one wanted to assist us as we didnt know the last number we dialed or the last amount we recharged. We do not have that info as we never had the chance to use the tablet. We literally phone Vodacom every day with out any success.
