Still being billed a year late

Hi,
I took out a contract and we changed it to a month-to month contract.
I cancelled this a year ago. We have continued to be billed to date, despite numerous calls to the call centre.
I was then told that we had a 3 year contract.
When I asked for a copy of this contract as the copy we have was clearly changed to month to month, they sent me a copy where the terms and conditions were unchanged and hadn't been initialled.
Despite repeated requests for the signed contract I still have not received anything and am still being billed.
I have spent the last week repeatedly phoning and being promised that a senior person would return my call but to no avail
