Incorrect debit order date

Netstar has requested for a debit order to go off on my account on the 07.06.2013 and again on the 18.06.2013 (WHICH IS NOT THE CORRECT DEBIT ORDER DATES)
The debit order was scheduled to run on the 25th of every month,
Why did the debit order not run on the 25th June 2013??

The debit orders were returned for the above dates requested in June 2013, resulting in me paying bank charges:

1. Please reveres all unnecessary bank charges on my bank account caused by incorrect debit order date
2. Request for the double payment to go off on the 25.06.2013
3. Advise why this error happened?
4. I have sent them numerous requests and no one has responded!
