Incorrectly debited my account after cancelling

I cancelled my premium DStv subscription, it was disconnected on the 29th May but Multichoice still debited R615 from my account in June and have still not refunded me a month later.

I phoned the call centre on the 7th May and asked for my DStv to be disconnected at the end of the month, and I was told by the person I spoke to that they had put in a request for my account to be deactivated on the 29th May so that there would be enough time to cancel the debit order before the beginning of June. On the 29th May our DStv stopped working, so I assumed all was in order. Then on the 3 June my account was debited R615.72 by Multichoice. I phoned a few days later to find out what was going on and explain that the debit was a mistake on Multichoice's part and they should refund me. I was told that the person I spoke to on 7th May hadn't correctly cancelled everything which was why the debit order had still gone through. I was told that a request for refund had been submitted and I must wait 14 working days for the refund. It has now been 17 working days and still no refund. Now the call centre says I have to submit bank statements for a refund - why wasnt I told this when I first called?
