Piece of equipment found in ice cream

Discovered a part of machinery / equipment (type of rotating blender blade) in Chocolate Vanilla Caramel ice cream while eating.
Would like to know how something like this happens as one could hurt oneself quite seriously should this piece of equipment happen to land up in your mouth.
