Claim for services not supplied to me

I received an e-mail from my medical aid that a claim from PATHCARE has been received an will be paid in 3 days. I have not received any service from PATHCARE!

How can they submit a claim without my autorisation?
How can they claim payment without supplying a service?

My Medical aid told me to call them, but how do I contact them if i do not know them?

Fedhelths response:
"Please note that the account for Dr Dietrich Bruce E Street & Partners was submitted electronically by the healthcare professional. The scheme is obliged to honor valid claims submitted to us by providers of service, therefore we cannot reverse any claim without their consent.
Kindly request your doctor to provide us with written confirmation in order to reverse the payments. This may be e-mailed to [Email Removed] to expedite your enquiry."

PATHCARE is not my doctor. FEDHEALTH and Pathcare are the only parties to this claim and they refuse to sort it out! Meanwhile my medical savings are by PATHCARE on a service not supplied to me!
