Incorrect billing and no response after a month

I was told at the end of last year that I could stay on Camaf and as per their brochure and costing structure the monthly premiums are based on earnings. As I am currently unemployed and traveling I was informed that I would need a signed affidavit from a commissioner of oaths. I gave them this as well as a letter from my HR department stating that my employment was ending at the end of December as an extra piece of evidence (one can only be employed if joining a medical aid and thus by proving that I am having my employment terminated and that fact that I am still a member of Camaf and not my 'new' employed.

My January invoice and debit order put me in the highest bracket. I emailed through my bank statement showing the only 'salary' coming in as a leave payout from deloitte relating to prior year.

About four emails later I am still yet to hear anything from Camaf and all of my emails are ignored. I have now once again been incorrectly billed for February again! I demand that I am reimbursed before I retire from Camaf who clearly cannot administer their business properly.
