Lesedi dragging their feet as always

I incurred an injury and was attended to at Lesedi Hospital on 15/12/2013, I was given a form J88 by the police for the Dr to fill out to support the incident I reported. The Dr said I should leave the form at the front desk and she will attend to it. I left the form at front desk and the lady said she would contact me as soon as the form was filled out and that I shouldnt come unless they call me. It's been nore than a month now and I am being perstered by the court for that form and nobody from Lesedi has bothered to call me. The service really.
