E-applying at discovery

Good day

I have been a member of Discovery health for over 7 years, i took a break for over 3 months in march this year, so i had to re-apply for a new membership which i dont have a problem with. I got the forms and sent them to [Email Removed] after some days i got a call from a broker indicating he needed me to update my information and send it back. I did so and he has never come back to me, i called today to follow up on my application and he didnt want to give me his name. Now my concern is i dont even know if he is a broker, how did he get my application when i forwarded it to [Email Removed] I really need my medical aid activated by the 1st of August and now im back at square 1. Does discovery not deal with applications, do they forward our details to brokers, and how can i just have my medical aid fixed without dealing with brokers
