Deceased, Motladi William Mfisa, id no. 6405055916089 worked for the company since 1996 December 15 and was dismissed on the 3rd of March 2011. Alexandra Forbes claimed that they made a payout to the deceased but on request for a proof, nothing was forthcoming, instead, a payslip was given which according to me does not constitute a provident fund payout proof. Company did not even pay for funeral expenses which according to law is covered by the fund. the fund denotes that a deceased should be paid 5 times his annual salary if he passes away still a member of the scheme, on that I still seek clarity.
