Nedbank NOT making it happen

I was issued an AMERICAN Express credit card by NEDBANK back in the early 1990's. I kept getting notifications to come and collect the card and responded each time, that I am not interested and do not want the card. Every month since then I have been paying R 22.00 service fees per month. Last month I received an SMS stating that the AMEX Card fees has once again been deducted of my account. I called NEDBANK and said to cancell the card and was told I had to visit my nearest branch
This is did and after explaining to 6 different people, I just wanted to close the card I had to call a call centre. I made it very clear, I do not even want the money they deducted from my account for the past number of year, I just wanted the account closed and I do not want an AMEX card.
I once again visited NEDBANK at Festival Mall in Kempton Park and once again had to tell my story to various people. Aventually, I walked out of tha bank and received a call from the Manager that was supposet o call me back. I even called her back and was put on hold - AGAIN.
Today I called AMEX directly and was put on HOLD - AGAIN.
All I want is to have the card cancelled - I never collected it, never used it
