No Communication regarding insurance polis

Hi.

Im writing a complaint for my mother. She is now above 60 years old and for years there is a Debit order of R22.80 being debited and she has no proof of this insurance nor can she remember what it was for. i need urgent response to the matter and also proof documents to be emailed to me regarding the insurance.
