Phonesured debit order decucted without consent

My fiance, Christo Vorster, had debit orders deducted for his Absa Savings account that he had not used for a long time and intended to close the account but found out that debit orders had been deducted for 10 months and it is a company called Phonesure for insurance on a cell phone (he had upgraded his cell phone 10 months ago)

We do not understand HOW they got hold of the banking details and how debit orders get approved without one even knowing of this. He has never signed an application form for insurance on his cell phone or approved of any debit order with them or with the Bank and obviously no specific cell phone is specified on this policy and no telephonic consent was ever given let alone the phone's emi number or make or model

We spoke to and got email response from Steven Zulu who said they will cancel the policy after he had listened to the call. We asked for a copy of the recording that was to determine whether he would be refunded. He also said it was company policy to request bank statements from us before this could be done and also wanted ID number

Half the amount has been refunded but not the full amount and we would like to have answers and
