Still waiting for account confirmation form!!!

I opened a new FNB gold cheque account telephonically on 01/11/2013, my new card was delivered on 13/11/2013 as expected. I expected my new card to be delivered with a welcome pack stating my new account details, however this has not been included. I urgently require a form stating my new bank details, my HR department requires this no later than 20/11/2013. I have left standard bank to join FNB because I have heard a lot of good things about FNB, please don't disappoint me. I am unable to visit a branch as I am extremely busy at work, I need this form to be emailed or faxed through to me. I have tried calling the call centre many times and I stayed on the line from 10 to 20 minutes today waiting for someone to answer. please contact me on 0782808399 or 0315333454 as this is quiet urgent, My company requires proof of my new banking details not just an account number telephonically.
