Incorrect deduction for travel insurance

I purchased a Swiss Air ticket for an international trip on my SAA American Express card on the 17th of July 2013. Chartis charged R539.00, to the same card, on the 19th of July 2013 for travel insurance purposes. This deduction is correct as I agreed that Chartis may deduct this amount should I purchase tickets for international travel on my card. On the 18th of October 2013, Chartis again deducted R539.00 for travel insurance even though I have not purchased another ticket for international travel on this card. I contacted Chartis on the 23rd of October 2013 at about 4:30pm and requested that they refund the second deduction of R539.00 and that they cancel the "automatic" debit-order on my card. I was given the following reference number - 231013167654235. To date, no refund has been made to my card. Once again I request that Chartis refund the second deduction of R539.00 and that Chartis confirm that they have cancelled the "automatic" debit order on my card.
