Account debited without authorisation

My bank account has been debited of an amount of R 399.00 for insurance I have not signed up for. I already have insurance with Unity insurance. I don't even have policy number or received a policy document and I have never spoken to any one from these people. The details I have put in the YOUR ACC/POLICY OR ID NO i s a reference that is on my bank statement.
