A costly income tax erro

In 2011 I was instructed by SARS to pay R81 189 because of an under-estimation of provisional tax for 2009. In addition, a review of tax returns was required for the years 2008, 2009, 2010.
How did this happen?
In 2009 my employer Old Mutual submitted an IRP5 certificate that reflected an income of R2.2 million (actual amount R25 890) for overtime duties. I had overlooked the error but was surprised that the firm Alliott Andersen Nell Inc. to which I had entrusted my tax returns, did likewise.
The error proved to be costly. The reviews amounted to R10472 (Alliott R5472, SARS R5000) and I became a high-risk tax payer. In the ensuing years I have had to submit documented proof of income.
Good manners dictate an explanation or apology. To date I have received neither, from Old Mutual or the Alliott group.
