Unfair deductions!!!

My September Premium didn't go of my bank account due to insufficient funds. my bank account was debited 3 times leaving me with an astronomical amount of bank charges. My October premium was due this month and the money was in my bank, but after i was told that i was not covered for September, the go and deduct my September AND October premium, and my October premium was not honoured due to insufficient fund. How can people deduct my September premium if i was not covered that month??? thus resulting my October premium not being covered.
